Born May 18, 1968 in Lancaster, Ohio to the late Wayne Notestone and Betty Senig Smith, Lancaster, Ohio.

Survived by his mother, Betty Smith, children, Amber (C.J) Frazier and Trevor (Katey) Notestone , grandchildren, Jaxon and Averi Frazier, sisters, Linda Upp and Barbara Hines, brother, Wayne Notestone.
